1. Purpose of the Two-Page Format
You have two separate pages (Page 1 and Page 2) that work together to record a thorough hoist examination:- Page 1 captures detailed checks on the overhead hoist (track, brackets, sling, etc.).
- Page 2 finalises the LOLER certificate, confirming safe use, label attachments, and signatures.
- This page has to be seperate to the service inspection.
2. Filling Out Page 1
- Equipment & Client Details:
Enter information like client address, hoist model, and serial number. - Inspection Sections (e.g., “Support & Reinforcement,” “Track & Brackets”):
- Choose “Complete (C)”, “Action Required”, or “N/A” for each main category.
- If you select “Action Required”, sub-points appear (for that category) so you can specify what’s wrong:
- Each sub-point can also be marked “Action Required,” “N/A,” or “Satisfactory.”
- If you pick “Action Required” at a sub-point, a detail box appears for notes and an “Action Completed?” option.
- Date of This Examination:
Once entered, the Next Examination Date automatically sets for six months later (by default). You can adjust if needed. - Weights Photo:
You must upload a photo verifying the load test. If left blank, the form reminds you before submission.
3. Moving on to Page 2
Certain key fields (e.g., job number, address, hoist make/model)copy themselves from Page 1 into Page 2.
This saves you time and helps keep records consistent.
4. Completing Page 2
- Auto-Filled Fields: Check that important details (job number, date, hoist info) transferred correctly.
- Next Exam Due & Last Exam: Make sure these dates are correct (often taken from Page 1).
- Hoist Condition: Mark whether the hoist is “Safe for Use” or “Unsafe for Use.”
- Labels: Confirm if a Service Label and a Battery Test Label have been attached.
- Signatures: Both Engineer and Customer must sign (and print their names) to approve the findings.
5. Submitting Both Pages
- Page 1: When you click. Submit, you’ll see a confirmation if all required details are filled and a valid photo is uploaded.
- Page 2: Submitting finalises the certificate. Any missing required fields (such as signatures) will prompt you before completion.
- Ideally the 2nd page will be created at the same time as Page 1 to save duplication by the engineer.
6. Why This Process Helps

Page 1 covers hoist checks in detail (with new “Complete/Action Required/N/A” logic for each category and sub-point);Page 2 focuses on the official certificate
